The README did not say whether the client can be pointed at a ClickHouse Cloud instance or a self-hosted one. Summary

This matches improvements already merged to other client library READMEs, e.g. ClickHouse/clickhouse-go#2075 and ClickHouse/clickhouse-java#3225.
